For many clients, living at home for long as possible is a priority. But the natural aging process can make that difficult over time. We provide in-home care services that can help prolong independent living. Diamond Heart Healthcare approach transforms day-to-day caregivinh into opportunities for meaningful connections. This includes conversation and activities that engage and enrich the physical, mental, social, and emotional lives of our clients. From helping with meals and mobility to personal care and errands, our in-home care lends a helping hand.
Private Duty Nursing.

· Colostomy care and urinary catheter maintenance
· Personal medical care including nail trimming, oral cavity cleaning, and bowel program supervision
· Administering medication including injections, inhalants, eye drops, oral, and topical medications
· Setting up and maintaining a medication schedule, including weekly pill boxes or automatic dispensers
· Tracking oxygen levels, use of heart monitors, or other advanced in-home equipment
· Providing advanced care training for family members
Our private duty nurses are available whenever you need them. From scheduled nursing visits to hourly care, someone is there to help 24 hours a day, seven days a week. We work with your loved one's medical team to offer the professional support he or she needs. Private duty nurses allow for loved ones to live at home while receiving the care they require. They complement traditional in-home caregivers and give family members extra peace of mind.

Diamond Heart Healthcare caregivers make our clients’ transitions from hospital to home as seamless, safe, and positive as possible. Our caregivers provide the following postoperative care services:
· Support for the care plan to aid in recovery after the surgery
· Assistance with household activities and responsibilities, including meal preparation, light housekeeping, and running errands
· Incidental transportation for postoperative and doctor’s appointments, such as office visits and physical therapy
· Communication with the medical team, as needed
Grooming, and Hygiene
Diamond Heart Healthcare goal is to assist our clients with their personal care routines, so they can look and feel their best. For clients who fear falling while bathing or have physical or cognitive limitations that make bath-time difficult, our caregivers are there to help. We can assist with shampooing, shaving (with an electric shaver), hair combing and styling, and other grooming needs.
Remaining active in your golden years is one of the best ways to maintain the quality of life. Our caregivers help seniors remain active by assisting them in getting from place to place in comfort and safety.
Transferring and Positioning
One of our top priorities is helping our clients move around their homes. This may include getting in and out of bed, chairs, and wheelchairs. Proper transferring and positioning can contribute to reducing bedsore risks. This also helps reduce muscle atrophy, while encouraging proper breathing and digestion. It can also lower the risk of falls and injuries, as well as keep our clients as comfortable as possible.
Toileting and Incontinence Care
We understand the sensitive nature of toileting and incontinence care for our clients. Without proper help, seniors can face many issues. These may include skin infections, unpleasant odors, and general hygiene problems. Our caregivers offer compassionate, dignified help with these personal aspects of daily living.
Maintaining a healthy, nutritious diet can impact well-being and vitality across the board. Out team provides companionship, turning meals into social time by engaging with seniors while they eat. The positive environment helps them feel more relaxed. Supervision also makes certain your loved ones get proper nourishment. Our caregivers also help clients eat nutritious menus to promote disease prevention, healing, and healthy living. We make an extra effort to turn meals into enjoyable social time by engaging our clients in conversation over food.
Although our caregivers cannot administer medications, they can assist in ensuring that clients take their medication as prescribed. Our caregivers can help with opening medication containers, reading labels, and reminding the client when it is time for the next dose.
It’s important for every caregiver to balance both the needs of the individual and their own. If this delicate dance is not performed correctly, it’s all too easy for a caregiver to “burn out,” leading to lack of sleep, imbalance in lifestyle, or overall decreased happiness.
